Guatemala officially mandated the reporting of births to the central government through the Civil Code of 1877. The National Registry of People (RENAP) has played a key role in strengthening birth registration, with the Organization of American States supporting RENAP's work to expand hospital registry offices and reduce under-registration.

While the Civil Code of 1877 mandated reporting, the system has evolved over time. Between 2011 and 2013, RENAP worked to establish 18 hospital registry offices, and by 2013, 73% of births in national hospitals were registered through this system. RENAP continues to expand its network, with additional offices installed in hospitals and social security network
