When you upgrade your MyHeritage DNA kit to receive the MyHeritage Health reports, you receive an e-mail (as seen below) with a button that will lead you to the questionnaire:
However, if you missed that e-mail or didn't even receive it, you can still access your questionnaire and fill it out like so:
1. Log in to your MyHeritage account by going to www.myheritage.com/login;
2. Hover over (but don't click) the DNA tab in the menu bar at the top of the page;
3. In the drop-down menu, click on Manage DNA kits;
4. Click on the red link next to your DNA kit;
5. Click on the blue link in the text;
6. Verify your password in order to access the Health questionnaire.
